Nutraferon ERTIGA 1 g Injection is a long-acting, Group 1 parenteral carbapenem antibacterial formulated with 1000 mg (1 g) of sterile Ertapenem (as Ertapenem Sodium). Engineered for inpatient care, nephrology, surgical prophylaxis, and Outpatient Parenteral Antimicrobial Therapy (OPAT), it delivers potent once-daily bactericidal activity against extended-spectrum beta-lactamase (ESBL)- and AmpC-producing Enterobacterales, Gram-positive aerobes, and anaerobes. Clinical Highlights

Bactericidal PBP2 & PBP3 Binding: Ertapenem penetrates the bacterial outer membrane and binds preferentially to penicillin-binding proteins PBP2 and PBP3 in Gram-negative rods, inhibiting peptidoglycan synthesis and causing rapid cell-wall lysis.
Concentration-Dependent Protein Binding: Reversible binding to human serum albumin prolongs systemic circulation, maintaining free drug concentrations above the MIC for >24 hours against susceptible Enterobacterales, streptococci, and Bacteroides fragilis.
Renal Elimination: Approximately 80% of the dose is excreted in urine (38% unchanged, 37% as ring-opened metabolite), requiring a 50% dose reduction (to 500 mg daily) when CrCl ≤ 30 mL/min.

Directions of Use & Safety Information

Professional Clinical Administration Only: For Intravenous (IV) infusion (over 30 minutes) or deep Intramuscular (IM) injection (for up to 7 days) by qualified healthcare personnel. Do not self-administer.
Critical Reconstitution & Diluent Rules (Pharmacist Alert):
- For IV Infusion: Reconstitute with Water for Injection or 0.9% Sodium Chloride and immediately dilute in 50 mL of 9% Sodium Chloride ONLY. NEVER dilute or mix Ertapenem in Dextrose/Glucose-containing solutions (causes rapid chemical degradation) or with other medications.
- For IM Injection: Must be reconstituted with 3.2 mL of 0% Lidocaine HCl injection ( WITHOUT epinephrine) and injected deep into a large muscle mass (gluteal or lateral thigh); never administer the lidocaine-reconstituted solution intravenously.
Renal Dose Adjustment: In adult patients with CrCl ≤ 30 mL/min or ESRD on hemodialysis, reduce the daily dose to 500 mg once daily (and administer a 150 mg supplemental dose if given within 6 hours prior to hemodialysis).
