Directions of Use & Safety Information

Professional Hospital Administration Only: Administered exclusively by qualified oncologists, physicians, anaesthetists, or registered nurses. Do not self-administer.
Standard IV Administration Rate & Dilution: For the 8 mg (4 mL) oncology dose, administer either via slow IV injection over at least 2 to 5 minutes, or preferably diluted in 50 to 100 mL of 0.9% Sodium Chloride or 5% Dextrose and infused intravenously over 15 minutes immediately before chemotherapy.
CRITICAL REGULATORY IV DOSE CAPS (Pharmacist Safety Alert):

Geriatric (≥75 Years) Single-Dose Cap: In patients aged 75 years and older, the maximum single IV dose is 8 mg (one 4 mL ampoule), which MUST be diluted in 50–100 mL and infused over at least 15 minutes (never given as a rapid IV push in ≥75 yrs!).
Adult (<75 Years) Single-Dose Cap: Never exceed 16 mg (two 4 mL ampoules) in a single IV infusion over 15 minutes due to Torsades de Pointes / QTc prolongation risk.
Severe Hepatic Impairment (Child-Pugh Class C): Total daily dose must NOT exceed 8 mg (one 4 mL ampoule) in 24 hours.
