Nutraferon NUTRACEF Injection

Many hospital and community bacteria produce β-lactamase enzymes that hydrolyze Ceftriaxone. Sulbactam permanently neutralizes Class A β-lactamases and adds intrinsic activity against Acinetobacter species.

Ceftriaxone has balanced dual clearance (~55% renal, ~45% biliary). However, because the Sulbactam component (500 mg) is 75–85% renally excreted and has a 1-hour half-life, in severe renal impairment (CrCl < 30 mL/min) the Sulbactam dosing interval must be adjusted by the nephrologist.

Ceftriaxone forms fatal crystalline precipitates with calcium in the lungs and kidneys of neonates. It is strictly contraindicated in neonates (≤28 days) who require (or are expected to require) any calcium-containing IV solutions within 48 hours, and is also strictly contraindicated in hyperbilirubinemic (jaundiced) neonates ( especially premature infants) because Ceftriaxone displaces bilirubin from serum albumin and can cause bilirubin encephalopathy (kernicterus).

Because 40–50% of Ceftriaxone is concentrated in the bile, it can bind biliary calcium to form reversible gallbladder precipitates ("gallbladder sludge" or pseudolithiasis) seen on ultrasound, which typically resolve upon stopping therapy.

Therapeutic blood and tissue levels are achieved immediately upon IV administration, with clinical defervescence and symptom improvement typically seen within 48 to 72 hours.
