Methylprednisolone is 5 times more potent as an anti-inflammatory agent than hydrocortisone while having negligible mineralocorticoid (salt- and water-retaining) effect—making it ideal when pure anti-inflammatory/immunosuppressive action is needed without fluid overload. (Conversely, Hydrocortisone is preferred in adrenal crisis where salt retention is needed.)

Rapid IV push (especially of large doses in less than 10 minutes) can trigger sudden cardiac arrhythmias, hypotension, or circulatory arrest. Always administer 40 mg slowly over several minutes and infuse larger doses over 30 to 60 minutes.

1) Contraindicated in systemic fungal infections and Traumatic Brain Injury (acute head injury) (increased mortality in the CRASH trial). 2) If a specific 40 mg presentation contains lactose derived from bovine milk, it is contraindicated in patients with known or suspected Cow's Milk Allergy (rare anaphylaxis reported with lactose-containing 40 mg methylprednisolone vials!). 3) Preservative-containing diluents (benzyl alcohol) are contraindicated in neonates.

Courses lasting more than 5 to 7 days suppress the hypothalamic-pituitary-adrenal (HPA) axis; gradual tapering prevents rebound inflammation and acute adrenal insufficiency.
