Unfractionated Heparin has a short 60-to-90-minute half-life, does not depend on kidney filtration for clearance at therapeutic doses, and can be completely reversed within minutes by intravenous Protamine Sulfate—unlike LMWH, which accumulates in kidney failure and is only partially (~60%) reversible.

Clinicians monitor the activated partial thromboplastin time (aPTT) or anti-Factor Xa level every 4 to 6 hours after initiation or dose adjustment until the target therapeutic range (typically an aPTT of 1.5 to 2.5 times baseline or anti-Xa of 0.3–0.7 IU/mL) is achieved.

HIT is a prothrombotic immune reaction (typically occurring 5 to 14 days after starting heparin, or sooner with prior exposure) where IgG antibodies bind the Heparin–Platelet Factor 4 (PF4) complex, activating platelets and causing life-threatening venous and arterial thrombosis alongside a >50% drop in platelet count. If HIT is suspected, stop all heparin immediately and switch to a non-heparin anticoagulant (such as argatroban or fondaparinux).

Heparin reversibly inhibits adrenal zona glomerulosa synthesis of aldosterone (heparin-induced hypoaldosteronism), which can cause clinically significant hyperkalemia within 3 to 7 days—especially in patients with Chronic Kidney Disease, diabetes mellitus, metabolic acidosis, or those taking ACE inhibitors, ARBs, spironolactone, or trimethoprim!

Stop the infusion immediately and administer slow intravenous Protamine Sulfate (approximately 1 mg of Protamine neutralizes 100 IU of Heparin remaining in the body, adjusted for time elapsed since the last heparin dose; max 50 mg over 10 minutes).
