Nutraferon TYGEE Injection

Tigecycline is a glycylcycline with a bulky 9-t-butylglycylamido side group that binds bacterial 30S ribosomes 5 times more tightly and blocks standard tetracycline resistance mechanisms, retaining activity against MRSA, VRE, ESBLs, and MDR Acinetobacter.

1) Tigecycline does NOT cover Pseudomonas aeruginosa, Proteus, Providencia, or Morganella. 2) It is NOT approved for Hospital-Acquired/Ventilator-Associated Pneumonia (HAP/VAP) or diabetic foot infections (failed non-inferiority trials with higher mortality). 3) Because it leaves the bloodstream rapidly to enter deep tissues, it achieves low blood and urine levels and should not be relied upon as monotherapy for primary bacteremia or UTIs.

No dose adjustment is needed in kidney disease or dialysis. However, because 59% of Tigecycline is eliminated via the liver and bile, patients with severe liver cirrhosis (Child-Pugh Class C) must have their maintenance dose cut in half from 50 mg to 25 mg IV every 12 hours (after the 100 mg loading dose).

Properly reconstituted Tigecycline is yellow to orange; if it turns green or black (due to oxidation), it must be discarded. Infusing slowly over 30 to 60 minutes helps minimize nausea and vomiting (the most common side effects).

Clinicians monitor for acute pancreatitis (including fatal cases—check serum amylase/lipase if severe abdominal pain occurs), coagulopathy / hypofibrinogenemia (monitor PT/INR, aPTT, and fibrinogen), hepatotoxicity, and Warfarin interaction (Tigecycline decreases clearance of R- and S-warfarin, increasing INR; monitor INR closely) and calcineurin inhibitors (tacrolimus/cyclosporine levels can rise).
