Unlike diuretics that remove fluid or calcium channel blockers that act directly on peripheral vessels, Clonidine stimulates alpha-2 adrenergic receptors in the brainstem's vasomotor center. It acts as a central "brake" on sympathetic nerve signals, reducing vessel constriction and heart rate at the neurological source.

Patients with Chronic Kidney Disease often exhibit heightened sympathetic nervous system activity, and some cannot tolerate ACE inhibitors or ARBs due to hyperkalemia or acute GFR drops. Clonidine lowers blood pressure without reducing renal perfusion or raising serum potassium.

Stopping Clonidine abruptly causes a massive rebound surge of adrenaline and noradrenaline, spiking blood pressure dangerously. Crucially, if a patient is also taking a beta-blocker (such as metoprolol or atenolol), abrupt clonidine withdrawal leaves alpha-adrenergic vasoconstriction completely unopposed, worsening the hypertensive crisis. Beta-blockers must be discontinued several days BEFORE gradually tapering Clonidine.

Dry mouth, drowsiness, sedation, mild constipation, and orthostatic dizziness (lightheadedness when standing up quickly) are common initially and often lessen after 2 to 4 weeks. Rise slowly from sitting or lying positions and avoid alcohol or sedatives.

Take the missed dose as soon as you remember unless it is nearly time for your next scheduled dose. Never double a dose, and never let your prescription run out over a weekend, as missing consecutive doses can trigger rebound hypertension.
