Tazobactam has higher intrinsic inhibitory potency against many Ambler Class A Extended-Spectrum β-Lactamases (ESBLs) produced by E. coli and Klebsiella, allowing a 125 mg dose (8:1 ratio) to protect 1000 mg of Cefoperazone, whereas Sulbactam (500 mg in `NEXON INJ`) is often favored when intrinsic anti-Acinetobacter activity is desired.

Yes. Because 70–75% of Cefoperazone is excreted via the bile and only 125 mg of renally cleared Tazobactam is present per vial, it offers a favorable renal profile, with interval adjustments required mainly in severe renal impairment (CrCl < 30 mL/min) or combined hepatorenal failure.

Cefoperazone's NMTT side chain blocks aldehyde dehydrogenase, triggering an acute disulfiram-like reaction (flushing, tachycardia, sweating, hypotension, and vomiting) if alcohol is ingested during treatment or within 3 days after the final dose.

The NMTT side chain of Cefoperazone interferes with Vitamin K-dependent clotting factor synthesis (Factors II, VII, IX, X), increasing bleeding risk in malnourished, elderly, hepatic, or anticoagulated patients. Prophylactic Vitamin K1 supplementation and PT/INR checks prevent coagulopathy.

Intravenous administration achieves peak bactericidal blood and biliary/peritoneal concentrations immediately, with clinical defervescence and leukocyte improvement typically observed within 48 to 72 hours.
