Nutraferon NEXON Injection

Sulbactam neutralizes bacterial Class A β-lactamase enzymes that would otherwise destroy Cefoperazone, while adding direct bactericidal activity against hospital-acquired Acinetobacter baumannii and anaerobic bacteria.

Unlike most cephalosporins that rely entirely on kidney filtration, 70–75% of Cefoperazone is cleared through the liver and bile. Only in severe renal failure (CrCl < 30 mL/min) is the dosing interval extended to account for the renally cleared Sulbactam component.

Cefoperazone contains an N-methylthiotetrazole (NMTT) chemical side chain that inhibits aldehyde dehydrogenase (the enzyme that breaks down alcohol), causing an acute disulfiram-like reaction (severe facial flushing, throbbing headache, sweating, rapid heart rate, hypotension, and vomiting) if alcohol is consumed during therapy or within 72 hours (3 days) after the last dose.

The NMTT side chain of Cefoperazone also inhibits hepatic Vitamin K epoxide reductase (similar to warfarin) and suppresses gut flora that synthesize Vitamin K. This can cause hypoprothrombinemia and bleeding—especially in ICU, malnourished, renal, or hepatic patients—requiring PT/INR monitoring and prophylactic Vitamin K1 (10 mg weekly) when indicated.

Cefoperazone concentrates in bile at levels many times higher than in blood, providing potent targeted eradication in acute cholecystitis, cholangitis, and biliary peritonitis.
