Clinical Highlights

Competitive Peripheral & Central 5-HT3 Receptor Blockade: Cytotoxic chemotherapy, abdominal radiation, and general anesthetics stimulate intestinal enterochromaffin cells to release serotonin (5-HT). Parenteral Ondansetron rapidly blocks 5-HT3 receptors on peripheral vagal afferent fibers and centrally within the chemoreceptor trigger zone (CTZ) of the area postrema.
Parenteral Pharmacokinetics: Following a 4 mg IV bolus, peak plasma concentrations are achieved immediately with a volume of distribution of ~140 L and an elimination half-life of 3.5 to 5.5 hours in adults (prolonged up to 20 hours in Child-Pugh C hepatic failure).

Directions of Use & Safety Information

Professional Hospital Administration Only: Administered exclusively by qualified physicians, anaesthetists, oncologists, or registered nurses via slow Intravenous (IV) injection (over at least 2 to 5 minutes), IV infusion (diluted in 50–100 mL of 0.9% Sodium Chloride or 5% Dextrose over 15 minutes), or undiluted deep Intramuscular (IM) injection. Do not self-administer.
Standard Adult PONV & CINV Dosing: For PONV prophylaxis/rescue, typically 4 mg (one 2 mL ampoule) undiluted IM or slow IV over 2 to 5 minutes at induction of anesthesia or post-operatively. For CINV, 0.15 mg/kg or 8 mg IV diluted over 15 minutes.
CRITICAL REGULATORY IV DOSE CAPS (Pharmacist Safety Alert): 1) No single IV dose of Ondansetron may ever exceed 16 mg (and in patients aged ≥75 years, no single IV dose may exceed 8 mg infused over 15 minutes) due to dose-dependent QTc prolongation and Torsades de Pointes risk. 2) In severe hepatic impairment (Child-Pugh C), the total daily dose must not exceed 8 mg/24 hours (maximum two 2 mL ampoules per day). 3) Do NOT mix with alkaline solutions (such as 5-fluorouracil, furosemide, or sodium bicarbonate), which cause precipitation.
