Traditional PPIs are acid-labile prodrugs that only bind actively pumping proton pumps and take 3 to 5 days to reach full strength. Vonoprazan is acid-stable, blocks the K+ channel of both resting and active proton pumps directly, and reaches maximal acid suppression on Day 1.

Because Vonoprazan binds both resting and meal-activated proton pumps and has a long residence time at the parietal cell canaliculus, its efficacy is completely independent of food intake.

Amoxicillin and clarithromycin require stomach pH above 6.0 so H. pylori bacteria actively divide and become vulnerable to antibiotics. Vonoprazan maintains pH > 6.0 far more consistently than standard PPIs, even in CYP2C19 rapid metabolizers.

Vonoprazan is STRICTLY CONTRAINDICATED with Rilpivirine-containing HIV medications (causes loss of virologic response due to elevated gastric pH) and significantly reduces absorption of Atazanavir, Nelfinavir, Ketoconazole, Itraconazole, Erlotinib, and Iron salts. Additionally, Vonoprazan is a weak-to-moderate inhibitor of CYP3A4 and CYP2C19 (can reduce Clopidogrel activation and increase Midazolam/Cilostazol/Tacrolimus levels), while Clarithromycin (a strong CYP3A4 inhibitor) increases Vonoprazan plasma levels ~1.5–1.8-fold.

As with all potent acid suppressants, prolonged use (>1 year) warrants periodic monitoring for serum magnesium (hypomagnesemia), Vitamin B12 status, fundic gland polyps, and Clostridioides difficile diarrhea risk, as well as elevated serum gastrin levels (which normalize within weeks of discontinuation).
