In surgical patients under general anaesthesia, patients who are nil-by-mouth (NPO), or oncology patients actively vomiting, oral tablets cannot be swallowed or retained. Intravenous injection provides immediate, guaranteed blood levels.

Chemotherapy and anaesthetic gases trigger intestinal cells to release a surge of serotonin that activates 5-HT3 receptors on the vagus nerve and in the brain's vomiting center. Granisetron blocks these receptors so the vomiting reflex cannot fire.

For chemotherapy or radiation, it is infused 5 to 30 minutes prior to the session. For surgery, the anaesthetist typically administers it shortly before anesthesia induction or immediately prior to the end of surgery, or as an immediate rescue injection in the recovery ward.

Granisetron can cause dose-dependent QTc interval prolongation; ECG and electrolytes (potassium and magnesium) should be monitored in patients with cardiac arrhythmias, congestive heart failure, or those receiving cardiotoxic chemotherapy (such as anthracyclines).
