Oral thiamine absorption relies on saturable intestinal transporters (ThTr1/2) that absorb at most ~4.5–8 mg from a single oral dose (and even less in alcohol dependency or malabsorption). Only IV or IM injection achieves the high blood concentration gradient needed for rapid passive diffusion into the brain.

Giving intravenous glucose to a thiamine-depleted patient forces their cells to burn glucose rapidly, consuming the last traces of Thiamine Pyrophosphate (TPP) and causing acute lactic acid buildup and permanent brain injury in the mammillary bodies and thalamus. Always infuse Thiamine in 0.9% Normal Saline first.

Crucial Biochemical Cofactor Pearl (Omitted in Original File): Magnesium (Mg2+) is an obligatory cofactor for both thiamine pyrophosphokinase (which converts thiamine into active TPP) and transketolase! Patients with alcohol dependency, malnutrition, or refeeding syndrome who have uncorrected hypomagnesemia may appear "thiamine-refractory" until Magnesium is simultaneously repleted!

Thiamine is a small, water-soluble molecule that is rapidly removed across haemodialysis and CRRT filters, while oncology patients with prolonged vomiting, mucositis, or TPN are at high risk of rapid 2-to-3-week thiamine store depletion.

Rapid undiluted IV push can rarely trigger burning at the vein site or anaphylactoid hypersensitivity reactions; diluting in 50–100 mL of 0.9% Sodium Chloride and infusing over 15 to 30 minutes maximizes safety.
